Hold Harmless Agreement
Determined 2B Present, Inc.
Upon registration for any course or service provided by Community CPR/BLS Connection, Determined 2B Present Inc., you hereby consent to the following terms and conditions:
Acknowledgment of Risks: You acknowledge your understanding of the potential risks and dangers associated with CPR, First Aid, BLS, PALS, ACLS, and all other instructional programs offered by Community CPR/BLS Connection, Determined 2B Present Inc. These risks encompass, though are not restricted to, personal injury, physical ailment, disability, and property loss.
Release of Liability: You agree to absolve and release Community CPR/BLS Connection, Determined 2B Present Inc., along with its proprietors, officials, representatives, and educators, from any and all liability, claims, damages, or injuries incurred as a result of your involvement in any of our courses or programs, irrespective of whether such claims arise due to negligence on the part of Community CPR/BLS Connection, Determined 2B Present Inc., its proprietors, officials, representatives, or educators.
Assumption of Risks: You accept full responsibility for all risks related to your participation in any course or program provided by Community CPR/BLS Connection, Determined 2B Present Inc., inclusive of, yet not limited to, the risks of personal injury, physical ailment, disability, and property loss.
Indemnification: You undertake to indemnify and hold harmless Community CPR/BLS Connection, Determined 2B Present Inc., its proprietors, officials, representatives, and educators, against any and all claims, damages, or injuries arising from your involvement in any of our courses or programs.
Medical Condition Disclosure: You affirm that you do not possess any physical or mental condition that would impede your ability to engage in any course or program provided by Community CPR/BLS Connection, Determined 2B Present Inc. Moreover, you agree to disclose to Community CPR/BLS Connection, Determined 2B Present Inc. any medical condition that might affect your participation in any course or program provided by Community CPR/BLS Connection, Determined 2B Present Inc.
Photography Release: You grant Community CPR/BLS Connection, Determined 2B Present Inc. the right to utilize any photographs or video recordings capturing your likeness during any course or program for promotional purposes.
By enrolling in any course or program offered by Community CPR/BLS Connection, Determined 2B Present Inc., you affirm that you have thoroughly read and comprehended this Hold Harmless Agreement, and that you are willingly and knowingly relinquishing any and all claims arising from your participation in any course or program provided by Community CPR/BLS Connection, Determined 2B Present Inc.
